How do I get an assessment?
Following the initial contact, we will arrange to meet with yourself, family or healthcare professionals. An assessment will allow everyone involved to have clarity of an individual’s needs.
Do I need a care plan?
The care plan allows documented agreements on goals and expectations. It ensures you get what is promised and agreed and allows future assessments to be evaluated for change.
What support can you find me?
Having an established network of experienced and reliable professionals aids us to quickly get you the support you need or second opinions. We also recruit and train for bespoke needs.
