What our Case Managers do
Our case managers are involved in completing detailed assessments, comprehensive case management plans and assisting clients, their families and support network to manage their neurological conditions in line with their personal goals.
Our case managers may also be involved in the recruitment process and management of client support teams, managing support workers by provision of training and supervision to ensure they are well informed about the client, their needs and are able to provide the best possible care and support in line with new legislation and best practice principles.
All our case managers prepare and maintain written records and produce reports, to monitor the client’s progress and care at agreed intervals as determined by the client or their representative.

Before we can provide a compressive care plan, we will arrange a professional healthcare assessment or review. Our assessments and interventions can take the form of one or all of the opposite.

Professional Referral & Support Interventions
In line with the Initial Needs Assessment (INA) and ongoing case management intervention it is important that the client is afforded appropriate access to specialist clinicians. This may take the form of referral to the following clinical disciplines below.
